FEES AND POLICIES

Session fees

questions to ask your insurance

questions to ask your insurance

Patients are billed directly, and payment in full is due at the time of service.  Although I am not an in-network provider for any insurance companies, services are often reimbursable through your insurance plan using out-of-network benefits.

After payment is made, documentation (in the form of a Superbill) will be provided to you and can be submitted to insurance for reimbursement.  The practice does not directly communicate with your insurance company.

questions to ask your insurance

If you are interested in seeking out-of-network reimbursement for your treatment, please contact your insurance provider to review your benefits.  Below are some helpful questions to ask them:

  • Do I have out-of-network mental/behavioral health benefits?
  • What is my annual deductible, and has it been met for this year?
  • How many sessions per calendar year does my plan cover?
  • What is my co-insurance amount?
  • What is the usual and customary rate for outpatient psychotherapy in my area (CPT code 90834)?
  • Is preauthorization required?

cancellation policy

I understand that there are unexpected changes in life, and there is no charge for cancellations made with 24-hours notice.  Whenever possible, I will work with you to reschedule your appointment.  If less than 24-hours notice is given and we are unable to reschedule your appointment, you are responsible for the full session fee.  It is important to note that most insurance companies will not provide reimbursement for missed session fees.
